looks like you did not fill everything in

you have nothing for ya data center. 1|city name,2|city name

you could use a drop down menu for your data center location, and not a text box

and in the "Select Options" put 1|city name,2|city name and more if needed

when i put it like this

in options its dosent show my ct name

its shows the number 1

have you tried changing the name of it instead of using data centre use location

 

Field Name: Location

Select Options: 1| whatever you would like to call your city
